The Painboy

The Painboy is the Ork medic of Warhammer 40k, a Mad Dok who puts Greenskins back together after a fight, usually while experimenting on them. He matters because he’s the closest thing Orks have to a doctor, and he’s also one of the scariest things on the battlefield.

What a painboy actually does

Painboyz are Ork Oddboyz who serve as the doctors of Greenskin society. They fix the injuries normal Ork toughness can’t handle on its own, like severed limbs, major trauma and brain damage.

Their two great loves are surgery and extreme dentistry, and they often mix the two. Teef are the standard Ork currency, so pulling teeth is both treatment and payment. They pick up their craft through a mix of genetic memory, trial and error and what Orks call ‘Orky know-wotz’.

Every Painboy is assisted by Grot orderlies. These little helpers fetch tools, hold things steady and get dragged along into battle too.

Why Orks fear their own doctor

Most Orks think Doks aren’t quite right in the head, and they’re not wrong. A Painboy will happily try experimental procedures on a patient who’s out cold, and the squig brain transplant is one of the most dreaded.

They also fit bioniks, though sometimes they get distracted and replace the wrong body part. Ork resilience usually saves the patient anyway, and there are Orks walking around after having arms, legs or even heads swapped.

Because of all this, an Ork only visits a Painboy when there’s no other choice. The painboy is the last resort, not the first stop.

Painboyz in battle

Painboyz are still Orks, so they join the fight with the same enthusiasm as anyone else. They swing power klaws and surgical saws, and they carry fungal serum syringes loaded with nasty stuff, including spores that make a victim’s blood congeal.

Their curiosity doesn’t stop at other Orks. On the battlefield they’ll capture enemies to study, performing vivisections without anesthetic. Some prisoners get turned into cybernetic monstrosities, and others are just taken apart because the Painboy felt like tinkering.

Ambitious Painbosses have even tried to capture Space Marines, hoping to unlock their secrets.

Variants and notable Doks

Not every Painboy is the same. Bad Doks are a darker, more unstable breed, Painbosses are the bigger and more ambitious leaders of the bunch, and Beast Snagga Doks work with the beast-hunting clans.

A few names stand out in Ork lore, like Dok Gutslash, whose bedside manner speaks for itself.

  • Bad Dok: a more dangerous and unhinged Painboy variant.
  • Painboss: a larger, more driven Painboy who leads others and pursues bigger experiments.
  • Beast Snagga Dok: a Painboy attached to Beast Snagga forces.
